Residential Treatment Centers:
Admissions
Criteria
LSS residential treatment centers serve
children with a variety of behavioral and psychological problems that interfere
with the children's day to day functioning and
limit their ability to live
in a less restrictive environment. Our admissions criteria include a psychiatric
diagnosis combined with severe behavioral problems, an IQ of 70 or above, and
for State of Texas referrals, a specialized or intensive level of care. We
typically only accept those children who have already received some form of therapeutic
or psychiatric services. We are currently unable to provide services to children
with mental retardation, sexually offensive behaviors, or a primary diagnosis
of substance abuse.
We treat children
between the ages of 6 and 17, although not all of
our facilities serve all ages. For admissions criteria

Application
Process
To apply to one of our Texas facilities,
please download an application
(pdf file) to fill out and send back
to us.
| You will need to include
copies of the following with your application:
• Psychological or psychiatric evaluation that has been completed in the
last 12 months
• Doctor's and/or medication orders and discharge papers,
signed by a doctor, if the child has been
hospitalized
in the last 12-16 months.
• Medicaid/Insurance Card
• School
Records (ARD and IEP if special education)
• Guardianship Papers (pertinent
legal papers) If the
child has been involved in CPS please include:
• Level of Care Assessment from Youth for Tomorrow)
• Placement
Log (if you have one from CPS) Copies
of the following forms are required prior to admission:
• Medical exam (within 30 days unless transferring from another agency)
• Dental exam (within the last year)
• Immunizations (all current)
• TB test (within the last year)
• School Withdrawal form
•
Social Security Card
• Birth Certificate |
