Care-Connect is a geriatric care-management program that offers families expert help with complex eldercare issues, coordinating medical and home-care services and providing emotional support. It is a blessing to both the loved-one and those responsible for their well being, especially when the responsible party lives in another city or state. Care-Connect provides loved-ones and caregivers peace of mind.
How It Works: n A specially trained geriatric care manager meets with the family and assesses the client's needs. n With input from professionals and others, a comprehensive care plan is developed. n The geriatric care manager then makes all the preparations for in-home care or transition to a care
facility.
n Once the care plan is put into action, all services are monitored and coordinated. Care plans are revised as necessary. n The geriatric care manager acts as a liaison to family members and
long-distance care givers.
