How It Works:
Our Geriatric Care Manager
1. completes a comprehensive assessment.
2. connects the client to needed services.
3. ensures all services are in place.
4. monitors those services.
5. reports to out-of-town family members if requested.
6. will continue services as long as needed, including facility placement and facility advocacy if that is necessary.
